DIRECTIONS FOR USING FORM:
1. Click the link above to open the form
2. Fill out the form, based on the information provided by your walk-in registrant. You will be filling out fields on the TOP HALF of the form. As you fill out the fields in the top half, the corresponding fields in the bottom half will populate.
3. Once all fields are completed, please click the “Print Form” button.
4. Cut or tear the form in half, in the middle (see dotted line). Give the bottom half of the form, “Event Ticket” to the walk-in registrant. Keep the top half of the form, “Counter Registration Form.”
5. Follow the directions in the “FOR OFFICE USE ONLY” section of the ‘Counter Registration Form’ to submit a copy of the form to Jackie Bailey (bailey.5@osu.edu), who will be keeping electronic copies of all those registered to attend the event.
